City4Age frailty and MCI risk model, v3 final (M36)Starting from the needs and unmet needs of the elderly population represented by the 6 Test Beds, this task will define a model of risks for frailty and MCI that will be the basic scientific rationale for the City4Age approach and the guidance in designing the approach to the problem and the interventions. It will consider several dimensions: including social, physical, mental. It will put together both the contributions from the geriatric science and the evidence coming from the organisations involved in the care process. This task will also consider the advances made in EIP AHA, in particular by the “commitments†involved in action A3 (Frailty) in understanding the various dimensions of the problem and the evidence gathered so far.
